The following are things I will require from you and questions I would like you to answer:

Pedigree

OFA  certificate

CERF certificate

Recent Brucellosis test that is negative

What is the age of your bitch?

How many times has she been bred?

When was the last time she was bred?

How many were in each litter?

Has she produced any genetic problems that you are aware of (dysplasia, eye problems, epilepsy, etc.)?

Are there any health problems? Allergies?  Sensitivities?

What problems do you know of in her ancestry?

Do you cull (euthanize) for anything?  If so, what do you cull for, when do you cull, and how do you cull? 

Do you require spay/neuter contracts on your pet puppies?

What do you require of your puppy buyers?

What would happen if you had to keep the whole litter?

What determines a pet quality puppy for you?  At what age do you make this decision?

What do you hope to gain from this breeding?  (What specific qualities or improvements are you hoping to achieve?)

What are your bitch's strong points?  Her weak points?
